Adult Adoption Questionnaire

Your answers in this questionnaire will give Randy the information he needs to: 1) fully advise you during your consultation, such as if there may be a legal problem and how to best overcome it; 2) if you retain him, to use this information to prepare your legal pleadings. Just click "submit" and it will be electronically delivered to Randy.

• The information you provide is confidential. If you do not retain Randy, the questionnaire will be destroyed.
• Randy will call you promptly to schedule your consultation. The consultation is about 30 minutes - is free - and you can elect to do it by phone or video (Zoom).
• Be aware that completing this questionnaire does not make Randy your attorney until you retain him and he agrees to represent you.
